A PCMH is an approach to providing comprehensive primary care for children, youth and adults.  The PCMH facilitates partnerships between individual patients, and their personal providers, and when appropriate, the patient’s family. The PCMH model has gained significant traction in the last few years mainly due to the focus and issues surrounding Healthcare Reform. PCMH concept was first introduced by the American Academy of Pediatrics in 1967 and today has evolved into established guidelines, coalition support, accreditations, and NCQA recognition.
